Does it help to resubmit my website to Google every once in a while?
First, let me tell you what you don't do. You don't use the "Add URL" form with Google at http://www.google.com/addurl.html. Believe it or not, using that form to let Google know about your site will actually hurt your PR and, at the same time, make Google rank you lower. In other words, you absolutely never want to use that form, or the similar form on any other search engine, to promote your website.
Can SEO companies guarantee high rankings?
Keep in mind that there are no guarantees when it comes to search engines. They don’t even guarantee that your site will be listed, let alone placed in the top rankings. With no ultimate control over the engines, we personally do not believe in misleading our own clients that we have such control by offering a guarantee for a particular ranking on particular search engines.
Google states "Beware of SEO's that claim to guarantee rankings" and we follow and agree with their guidelines on this.

Upto 5.
Number of key words targeted?
Upto 10. IE. the phrase "Cincinnati Wedding Photographer" would count as 3. "Cincinnati Wedding Photography" would bring it to 4. (Photography is the new word) The narrower the number of keyphrases we optimize for, the better chance we we will score on the most important keyphrases. 6-8 seems to be an optimum number to focus on.
